Pre-launch program materials. Dream Home Fund's Dream Simple program is in pre-launch. The templates on this page are operational reference for the program and may be revised before first borrower closings. Final versions distributed to enrolled lenders are the operative documents. Dream Simple closings begin upon HUD-Approved Nonprofit Roster approval and execution of the Lender Participation Agreement.

These templates supplement the Lender Program Manual and the Dream Simple Program Handbook. Loan-document instruments (DPA Note, Subordinate Deed of Trust) are distributed to enrolled lenders separately. To request additional materials or ask questions, email partners@dreamhomefund.com.

Secure transmission required. Lender submissions containing borrower personally identifiable information must be transmitted via Dream Home Fund-designated secure channels (encrypted file attachment with separate-channel password, Lender-controlled secure file-share link, SFTP, or Dream Home Fund's submission portal once designated). Unencrypted email attachments containing borrower PII are not permitted. See Lender Program Manual Section IV - Submission Method for full details.

Lender Participation Agreement (LPA)

The binding contract between Dream Home Fund and each enrolled lender. Establishes representations, obligations, audit and indemnification framework, and termination provisions.

The Lender Participation Agreement is the binding contract between Dream Home Fund and each enrolled lender. It incorporates the Lender Program Manual, the Dream Simple Program Handbook, and all other Program Documents by reference and establishes the parties' representations, obligations, audit and indemnification framework, defect / cure / repurchase remedy, and termination provisions.

Structure of the LPA

  1. Recitals and definitions. Identifies the Parties, recites Program purpose, and defines key terms aligned with the Manual and Handbook.
  2. Enrollment and authorization. Non-exclusive grant of right to originate; annual recertification requirement; effective on receipt of all qualifying items.
  3. Incorporation of Program Documents. The Manual, Handbooks, Lender Forms, and program bulletins are binding on the Lender to the same extent as if set forth in full.
  4. Lender representations and warranties. Organizational reps, licensing, compliance with Applicable Laws, no material litigation, insurance, BSA/AML and anti-fraud controls, OFAC screening, information security (GLBA / FTC Safeguards), and per-file representations restated on every submission.
  5. Dream Home Fund representations and obligations. Authority, administration commitments, review and determination targets, funding mechanics, and no-guarantee-of-funding limitation.
  6. Lender operational obligations. Compliance with the Manual; use of approved submission channels; borrower disclosure delivery; no independent solicitation; cooperation with DHF investigations.
  7. Submission, closing, and funding. File-level rep reaffirmation; closing conditions (2nd lien position, hazard insurance loss payee); Funding Authorization process; post-closing delivery within 10 business days.
  8. Audit rights. DHF audit on 10 business days' notice; 7-year file retention; cost reimbursement if Defects exceed a stated threshold.
  9. Confidentiality, information security, and privacy. Mutual confidentiality; PII handling per GLBA / FTC Safeguards / CCPA; 48-hour incident notification window.
  10. Indemnification. Lender indemnification scope including breach, negligence, file Defects, third-party Borrower claims, and PII incidents; mutual DHF indemnification; capped exposure on standard breaches with carve-outs for fraud, willful misconduct, confidentiality, PII, and Borrower claims.
  11. Loan Defects, cure, and repurchase. Definition of Defect; Notice of Defect process; 60-day cure period; election between repurchase and indemnification; Capital Partner coordination.
  12. Term and termination. 1-year initial term with automatic renewal; termination for convenience on 60 days' notice; termination for cause provisions; survival of confidentiality, audit, indemnification, and repurchase obligations.
  13. Insurance, force majeure, governing law (California), mediation, and venue.
  14. Notices, miscellaneous, signature page, and Schedules A-D (Licensing, Disclosed Regulatory History, File-Level Reps, Notice Addresses).

Lender Enrollment Application

Intake form for prospective lenders. Submitted before execution of the Lender Participation Agreement.

This is a starter intake template for prospective lenders wishing to enroll with Dream Home Fund. It is not a binding document on its own; execution of the Lender Participation Agreement (LPA) is the binding step. Intake responses below should be reviewed by Dream Home Fund staff before issuance of the LPA.

Lender Information

Legal company name, DBA, entity type, state of organization, year founded, EIN, primary office address, mailing address, main phone, website URL, NMLS Company ID, number of branches, approximate annual originations (units / volume), approximate first-mortgage product mix, TPO usage and count.

Primary Contacts

Designated Program Contact (operational): Name, title, email, phone.

Executive Sponsor (authorized signatory of the LPA): Name, title, email, phone.

Compliance / Quality Control Contact: Name, title, email, phone.

State Licensing

List each state in which the Lender is licensed to originate residential mortgage loans: state, license type, license/registration number, expiration date.

Regulatory History (last 3 years)

Disclose whether the Lender has been the subject of any of the following during the prior 36 months: formal regulatory enforcement action, suspension or revocation of any mortgage lender license, cease and desist order, material consent order, civil or criminal action related to mortgage origination practices, material findings in any GSE, HUD, or investor audit, lender repurchase demands (over 1% of originations), bankruptcy filing or assignment for the benefit of creditors. If any answer is "Yes," attach a written explanation including action taken to remediate.

Insurance Coverage

Per Section III of the Lender Program Manual, each enrolled Lender must maintain at all times: Fidelity Bond (or Direct Surety Bond), Errors and Omissions, Forgery insurance, and Money and Securities coverage. Provide carrier, policy number, coverage amount, and expiration date for each, plus certificates of insurance.

Financial Standing

Most recent audited or reviewed financial statement (or substitute acceptable to Dream Home Fund). Most recent fiscal year end, total assets, total equity, net income, auditor.

Program Interest

Which Dream Home Fund DPA programs does the Lender intend to offer? Dream Simple (available upon HUD approval).

Lender Certification

The undersigned, an authorized signatory of the Lender, certifies that all information provided is true, complete, and accurate; authorizes Dream Home Fund to verify the information; acknowledges that this application does not constitute approval (which requires execution of the LPA); and agrees to notify Dream Home Fund of any material change.

Signed by Executive Sponsor, with printed name, title, and date.

Lender Submission Checklist

One-page cover sheet submitted with each Dream Simple loan file. Captures lender contact info, borrower and transaction summary, qualifying data, document inventory, and lender certification.

Required cover sheet for every Dream Simple submission.

Section 1: Lender Information

Lender legal name, NMLS ID, submitting loan officer / underwriter (name, email, phone), and lender's preferred contact for conditions or corrections.

Section 2: Borrower & Transaction

Borrower(s) and co-borrower(s) full legal names; subject property address, city, county; property type checkbox (SFR detached / attached / condo / PUD / 2-4 unit); purchase price; expected closing date; occupancy.

Section 3: Loan Structure

First mortgage program (FHA, Conventional, VA, USDA, HUD 184, or other); first mortgage loan amount, note rate, and term; Dream Simple DPA amount requested; CLTV (must not exceed 105%); other subordinate financing.

Section 4: Borrower Qualifying Data (Informational)

Representative credit score, front-end and back-end DTI, AUS finding (DU Approve/Eligible, LP Accept, GUS Accept/Eligible, or Manual UW), first-time homebuyer status, HUD-approved homebuyer education completion date, Dream Home Fund Financial Education Course completion date, and confirmation of Reg B demographic monitoring at 1003.

Section 5: Documents Included (Checkbox Grid)

Checkbox inventory of the 17-line document list set out in Lender Program Manual Section IV: 1003, AUS findings, tri-merge credit, paystubs, tax returns, bank statements, purchase agreement, appraisal, title commitment, hazard insurance, flood determination, homebuyer education certificate, DHF DPA Application + Borrower Disclosure + FTHB Affidavit, DHF Financial Education Course certificate, and lender first-mortgage final approval.

Section 6: Lender Certification

Lender certifies (1) the submission is complete and accurate, (2) the Borrower(s) have been credit-qualified under the applicable first-mortgage program, (3) to the best of the Lender's knowledge after reasonable due diligence the Borrower(s) meet Dream Home Fund's program eligibility, (4) ECOA / Reg B demographic monitoring was collected at application, (5) the Lender is not aware of any fraud, misrepresentation, or unresolved compliance issue, and (6) the Lender acknowledges Dream Home Fund's rights under the Lender Participation Agreement to request additional documentation, re-verify, demand cure / repurchase / indemnification, and audit the file post-closing.

Signed by the submitting underwriter or authorized lender representative, with printed name, title, and date.

DPA Application, Borrower Disclosure, and First-Time Homebuyer Affidavit

Combined borrower-facing document executed by each borrower and counter-certified by the Lender. Replaces three separate documents (application, disclosure, affidavit) with one signed package.

A single borrower-signed document combining the Dream Simple DPA Application, plain-language Borrower Disclosure, and First-Time Homebuyer Affidavit, with a Lender certification in Part 4.

Part 1: Application

Borrower(s) full legal name, mailing address, email, phone, date of birth, and citizenship/residency status (U.S. citizen / Qualified Alien); subject property address; property type; purchase price; expected closing date; first mortgage lender, program, amount, rate, and term; and Dream Simple DPA amount requested.

Part 2: Plain-Language Borrower Disclosure

Plain-language explanation of what Dream Simple is and how it works: loan sizing (3.5% of price, capped at $45,000); no monthly payments; simple interest accrual at first-mortgage rate plus 1%; repayment triggers; refinance rules (rate-and-term allowed with subordination, cash-out triggers payoff); the silent second is not forgiven; the $500 Program Administration Fee; illustrative accrual example at $800,000 purchase price; Dream Home Fund's right to update the Handbook with grandfathering of more-favorable changes; ECOA Equal Credit Opportunity Notice; complaints escalation path; privacy and information use.

Part 3: Borrower Affidavit and Certifications

Borrower certifies under penalty of perjury: first-time homebuyer status with three-year lookback (with full definition of "ownership interest"); owner-occupancy intent within 60 days; U.S. citizen or Qualified Alien residency; HUD-approved homebuyer education completion; Dream Home Fund Financial Education Course completion; no cash back from Dream Simple funds; acknowledgment of repayment terms and accrual examples; truthfulness of all application materials; right to acknowledge.

Part 4: Lender Certification

Lender certifies first-mortgage credit qualification, completeness and accuracy of the submission, no known disqualifying facts, ECOA / Reg B demographic monitoring collection, and rep-and-warranty status under the Lender Participation Agreement.

Notary Acknowledgment

The borrower affidavit certifications are notarized using a California All-Purpose Acknowledgment, executed at the same closing table as the DPA Note and Subordinate Deed of Trust.

Annual Recertification

Each year, enrolled lenders submit updated licensing, insurance, financials, and regulatory history.

Per Section III of the Lender Program Manual, enrolled Lenders are subject to annual recertification. Each year on the anniversary of the Lender's LPA effective date (or such other date as Dream Home Fund may specify), the Lender shall submit a completed Annual Recertification.

Section 1: Lender Information Update

Confirm or update the information on file: legal name, DBA, primary office address, main phone, website, NMLS Company ID.

Section 2: Primary Contacts

Confirm or update the Designated Program Contact, Executive Sponsor (LPA signatory), and Compliance / QC Contact.

Section 3: State Licensing Update

Attach an updated list of all states in which the Lender is currently licensed. Disclose any new approvals, surrenders, suspensions, or revocations during the prior 12 months.

Section 4: Regulatory Events (past 12 months)

Disclose any formal regulatory enforcement action, license suspension or revocation, cease and desist order, material consent order, civil or criminal action, material GSE/HUD/investor audit findings, lender repurchase demands over 1%, bankruptcy, change in control, or material adverse change in financial condition during the prior 12 months.

Section 5: Insurance Coverage Update

Provide current certificates of insurance for Fidelity Bond, Errors and Omissions, Forgery, and Money and Securities coverage.

Section 6: Financial Standing

Attach the most recent audited or reviewed financial statement. Disclose total assets, total equity, net income, and any material change in net worth from the prior year.

Section 7: Dream Home Fund Origination History (past 12 months)

Summary of activity during the recertification period: number of silent seconds submitted, approved, funded, denied or withdrawn; total funded principal; number of post-closing defects; number of repurchase or indemnification requests.

Section 8: Third Party Originator Update

If the Lender uses TPOs to originate Dream Home Fund silent seconds, attach a current list including legal name, NMLS Company ID, state licensing footprint, and effective approval date.

Section 9: Lender Certification

Authorized signatory certifies that all information is true, complete, and accurate; that the Lender remains in compliance with all program requirements and the LPA; authorizes Dream Home Fund verification; and agrees to notify of any material change.

Loan-document instruments executed at closing (DPA Note, Subordinate Deed of Trust, Substitution of Trustee and Full Reconveyance) are distributed to enrolled lenders directly.
